Population Health

Our Population Health Team works across multiple specialised streams in nursing and health promotion to improve health outcomes for Aboriginal communities across the West Kimberley. Our focus is on health promotion, chronic disease prevention, and empowering individuals, families, and communities to make informed, healthier lifestyle choices. Through outreach, education, screenings, and behaviour change strategies, we aim to reduce the burden of chronic disease and strengthen long-term wellbeing for our people.

A core part of our work is preventing and managing chronic disease. The Chronic Disease Clinic at DAHS has grown significantly, now staffed with two GPs to provide consistent follow-up and build long-term care relationships. This expanded capacity allows for better continuity of care, improved patient engagement, and more effective management of complex chronic illnesses in a culturally safe environment.

Engaging with the community is central to our approach. The team actively participates in events like the KALACC Festival, providing opportunities to connect respectfully with local Elders and families, share health information, and promote wellbeing in ways that align with cultural values. Local education events also raise awareness about kidney health, early detection, and chronic disease prevention, giving community members the tools to take proactive steps for their health.
