Chronic Disease Care

Derby Aboriginal Health Service provides coordinated care to support people living with chronic health conditions.

Our chronic disease program focuses on prevention, early detection and long term management of conditions such as diabetes, heart disease, kidney disease, high blood pressure and other long term health concerns.

Care is delivered through a team based approach, with health professionals working alongside doctors and other clinic staff to support regular monitoring, follow up and ongoing care.

 

Supporting long term health

Living with a chronic condition can require regular check ups, tests and lifestyle support. Our team works with clients to help them understand their condition, stay on track with care and access the support they need.

This may include reminders for health checks, support to attend appointments, and referrals to other services where required. Wherever possible, care is provided in a way that fits around individual needs, family responsibilities and community life.

Screening and monitoring

As part of chronic disease care, DAHS supports regular health checks and screening to help detect changes early and reduce the risk of complications.

This includes monitoring related to conditions such as diabetes and heart disease, with appropriate referrals made to specialist services when needed.

The chronic disease program is designed to support people over time, building confidence and knowledge so clients can take an active role in managing their health.
